Mary Rose was nine when she first noticed a dry, cracking rash on her hands. Her father, a surgeon, said it was “just eczema’ and gave her antihistamine pills to relieve the itching.A week later, the rash flared so dramatically that her mother took her to an allergist. Subsequent probing and testing showed that Mary Rose was allergic to milk and also to the antihistamines. The treatment was making her worse! When she stopped the pills and went on a milk free diet, her rash subsided.
Eczema or atopic dermatitis (AD) follows no format or formula. It can appear at any age, although it’s most prevalent in babies and children. It can affect any area of the body but usually starts on the face or over elbows and knees in babies. About 85 percent of infant eczema cases clear spontaneously during childhood. Adults are more likely to find patches on the arms, legs, feet, scalp, behind the ears, and especially on the hands.
If YOU Suspect You Have Eczema Take These Steps:

Check with your doctor to rule out possible causes for the rash other than allergy.
*
Be on the lookout for all irritants and substances, including foods that seem to cause burning, itching, redness, or dryness.
*
Prevent skin dryness. Bathe or lubricate your skin once or twice a day. Use warm (not hot) water, minimal soap, and no bubble bath (which may contain irritants).
*
Moisturize with nonirritant cream, vegetable oil, or petroleum jelly (Vaseline) within three minutes after bathing and as often as necessary to keep skin from becoming scaly. (Moisturizers work best on wet skin.)
*
Avoid lotions that contain water and alcohol; they have a drying effect. Greasy ointments are better absorbed than creams.
*
Minimize contact with household cleaners, dyes, chemicals, body care goods, fragrances, cosmetics, and products such as jewelry or watchbands that might contain nickel. Lanolin, an oil obtained from sheep, and Quarternium-15 a preservative in shampoos and lotions, are also frequent sensitizers.
*
Wear gloves for dishwashing and household chores, but be wary of latex, which often causes irritations. (Recent FDA figures show that more than 10 percent of health, dental, and surgical workers are sensitive to latex. Powdered gloves are especially bad, as powder carries the allergens into the air. Some health care workers are now wearing vinyl gloves over and under the sandwiched latex glove.) If you notice redness, itching or irritation under your latex gloves, switch to vinyl.

For mild itching, ask your doctor or pharmacist for an OTC anti itch medication. “Calamine doesn’t do much,” says dermatologist Dr. John H. Epstein. “If the rash is weeping, you can use cool water compresses.”

Use medication immediately after a bath or a soak to increase absorption.
*
Wear loose, lightweight cotton clothing; no heavy wools or synthetics.
*
Change or rotate the brand of laundry detergent you use.
*
Trim your nails. Scratching can lead to infection.
*
Avoid extremes or rapid changes of temperature.
*
Avoid strenuous activity or exercise. Sweat can aggravate the rash.
*
Recognize and reduce stress. It leads to itching.
If eczema is confirmed, your doctor may suggest an OTC topical corticoid ointment, or a stronger, prescription cortico steroid (CCS) cream. These are potent drugs; 1.4 percent of patients become allergic to the creams and ointments themselves.Follow instructions: apply sparingly, rub in well, and don’t use medication prescribed for one rash on the next skin irritation. Short term use should cause no problems, but long term use of topical steroids can cause thinning of the skin and easy bruising.
